If you do NOT want to take part in this project, you MUST inform us of this, as this is an opt-out project and if you have not told us that you do not want to take part, then it will be assumed by HSCIC (Health & Social Care Information Centre) under section 251 of the NHS Act 2006, that you are happy to take part and have details of your annonymised prescription, referral, clinical and demographic information be shared with the HSCIC to allow them to review services, and treatment. Only data from April 2013 will be shared. Data before this date will not be shared.
